To answer your points about the use of titles inside the U.S.: they can call themselves anything they like here as a personal preference or unoficially (in the U.S.), but not on any official document of any type and at any level (national/state/local). The Constitution explicitly prohibits that formal recognition.

It doesn't matter if they hold office or not. They will still not be issued any document in their name by an federal or state authority that bears an heriditary title in addition to their given name. So let me ask you in turn: Why do you think it's so unusual that in a republic there are explicit legal clauses against granting of nobiliary titles, when in fact even in some existing monarchies there are no more nobiliary titles being granted anymore outside the immediate royal house? The general truth is this: if a sovereign nation either monarchy or republic decides to not allow hereditary titles, they have the authority do so. On the other hand, if their constitution or laws are totally "silent" about that issue, then that silence can be interpreted as possibly allowing it. So...it's one of those legal situations where the law must be explicit, or "silence" can be interpreted in more than one way. The head of state (monarch or president) is the fons honorum, and in theory or even practice has the option to grant a title...unless his/her constitution or legal provisions prevent from so doing. The framers of the American constitution decided not to be "silent" on this particular issue of hereditary titles, and their wording can be read as not only prohibiting the granting of such titles by any American authority federal or lower, but also prohibiting the legal recognition inside the United States of titles that were granted by external foreign authorities to American citizens.
