As an American citizen born in the United States, a foreign title will not, of course, appear on an American birth certificate as applied to that individual. American governing/vital documents can inform how other countries might use an individual’s name but they do not restrict it as such. One can have different names in different jurisdictions or even vital documents with different names in the same jurisdiction.
The Duchess of Sussex, for instance, is an American citizen with a passport bearing the name Rachel Meghan Markle but is still the Duchess of Sussex outside of the United States and His Majesty’s realms (and has been recognized as such).

By the way, the only actual BEARER of that document is in fact the individual Lilibet, the secondary REFERENCE to her father, a non-American person, is still NOT any recognition of that title being valid on any document issued to him PERSONALLY by any U.S. legal entity, federal or lower level.
