They are similar to any other decoration. They are also awarded to foreign Heads of State when they visit the Cortes on a state visit.
And, actually, they are worn in relevant ceremonies, like the opening of the Cortes. A picture of 2019.

I don't quite understand the siginficance of these, however. Are they gifts from the two chambers of the parliament? Why do the King and his heir receive these, when they are never (as far as I can recall) worn? Are they more symbolical. like the 'keys to the city' that royals occasionally receive? Previous Message

Yesterday, having reached her majority, Princess Leonor swore allegiance to the constitution and loyalty to the King before the Cortes
A beautiful ceremony, except the speech by the President of the Cortes, which was pathetic
The Princess wore a white suit and the miniature of the Golden Fleece.

After the ceremony at the congress, another ceremony at the Royal Palace, where she was invested with the collar of the Order of Carlos III. The Prime Minister, as chancellor of the Order, passed the collar to the King who, as Grand Master of the Order, invested the Princess.
Later, the Royal Family saluted the guest to a luncheon at the Throne Room.
