In the recession when our daughter was mostly unemployed we ended up paying for her medical insurance which was about $700/month then. If she got sick they would try to deny her the next year. It wasn't Obamacare, just the lack of mercy by insurance companies.
Yes, other countries have national medical coverage for all, and doctors and clinics charge accordingly. If your well off you have more choice by going outside of the system, but that's your choice.
My husband developed an infection in Malta. Fortunately the only hospital was next to our hotel. He went to the walk in clinic, saw a doctor and was quickly diagnosed. The fee was not very much even though we're not part of their insurance system. It was so low that he didn't put in for reimbursement from our travel insurance. He got the medicine there too for not very much.
