I am a provisional member. I have 3 sweepstakes in the last 5 years and just completed my second judging requirement, I also attended the workshop in the last four years. Is it true that I need to submit another application to attain the active membership?

Yes, you have to do the paperwork for it. But they should get it approved and you'll be on the active list once you do so!
