Then, I would finish class, after class I would make the student watch me fill out a write up form for them, and make a quick copy of it. I would ask him to take the copy home, and get it signed by his parent. If it wasn't returned the next day signed, I'd turn the original in to the office and call home.
The best advice I can give you, is don't try to correct behavior like that. Remove behavior like that from your environment. Then, try to prevent it from happening again. As a director who had high blood pressure before he turned 30 (thanks to the marching band diet and trying to deal with that type of behavior), I can tell you it's not worth your health to lose your cool, and it rarely has any positive long term effects.
Also, a lot of students don't respond appropriately if you call them out in front of the class. Talk to them one on one, it'll go smoother.
