Although tulips are perennial, they either start producing smaller and smaller blooms, or die after about 3-4 years. The ones that live longer than that, often need to be divided, to keep flowers a normal size. Critters are also responsible for "disappearing" spring bulbs, too. My next door neighbors, on both sides, now have a few unplanted spring bulbs, that got there, either from my plant seeds, or bulbs that were transplanted there, by squirrels. So, that said, I think it is normal for tulips to bloom, and then not bloom anymore, after a few years. I plant Some Dutch bulbs almost every year. I didn't plant any, last fall, but likely will, this upcoming fall.
I doubt that your bulbs were planted too deep. In fact, in the Far North, places like Minnesota or North Dakota, it is recommended that the normal 6 inch depth, be increased to about a foot, simply to help the bulbs survive the brutal temps these places get to, in winter.
