If there is no Consort than the first adult in the line of succession would be regent.
In case neither exist than a regent (1) or a regency council (3 or 5) can be appointed by the Cortes.
So in your scenario Queen Letizia would automatically be regent. If she has died or if she and the King would be divorced than the Infanta Elena would be regent.
