Start by stopping the corn and bread, there's little to no nutrition there. Check your feed, how old is it? The fresher the more nutrition they're getting.
I don't feed layer to my males, actually I don't use layer at all. Layer is hard on the males so I use a flock raiser type feed and have free choice calcium out for the girls to eat as they need it.
If this doesn't do it then there is only two things going on, something in your flock being passed through the eggs or something is wrong with incubation. You shouldn't have that high of a failure rate with your own eggs or that high of a die off once hatched.
