Acromelagy is a result of an over active pituitary gland. Depending on their age when the gland malfunctions, people either display extraordinary long bone growth (e.g. Robert Wadlow, the most famous "pituitary giant") or the characteristic heavy features of acromelagy (giantism). Richard Kiel, with his large lower jaw, heavy brow and enormous hands, appears to have relatively mild acromelagy. The cyst on his forehead in the Bond movies adds to the effect, and is reminiscient of the unusual bone growths of more severe cases. Another thing is that Kiel is broad, looking like a scaled up person, whereas pituitary giants can appear disproportionately thin.
I am no expert by any means, I just read a book on the subject.
