It could be used in a global sense to include all fanfics/manga with male-male relationship, or it could be used in a specific sense to indicate the 'doing' part only. Not all doujinshi are "yaoi" (or "yuli"). In addition, not all books are "yaoi" even they are made by the same circle. Simalar names also include "BL" (Boy's Love), or "June" (which is the first major boy's love novel magazine). Most, if not all Japanese yaoi authors are females.

The name "Yaoi" comes from 'YAma-nashi, Ochi-nashi, Imi-nashi' (no climax, no point, no meaning). It is a word used by Japanese douji world only (so you won't be able to find it in a Japanese dictionary), which is used to denote all sex-related male-male relationships. (Female-female is called "Yuli".)
