You would not call a close friend [first name]-chan when you address her to the teacher, for example. Sometimes it would be [the first syllable or the first two syllable of the first name]-chan, there are certain ways to short the specific first name. For example, "Kayura" maybe "Kayura-chan" or shorted as "Kayu-chan". In formal situations, or address friends that's older or not as close, it would be [first name]-san with a female. [Last name]-san would be used to address a female who is not as close.
It depends on your ages, the specific relationship (e.g., in school, at work, when do you know each other, etc), and how close you're to each other.
