Rebate Links 2003

I must have lost the rebate slip that told me the address of where to send the $15 rebate information. I recall the need for the title page of a previous Links Manual, sales receipt, and Links 2003 barcode.
Does anyone know where I should mail the rebate to? The rebate address is surprisingly not listed on the Microsoft site.
