I used a spin index fixture that indexes 1 degree for 360 degrees. I set the cam blank up using a mandrel and a support for the other end. I used a .200 dia end mill and indexed the spin fixture 1 degree at a time. Before indexing each time I may have had to move my milling table in the left/right direction using a dial indicator. I used an Excel spread sheet to list the movements for each degree for 360 degrees. My cam was from one of the original plans. But it worked out very nice with no hack sawing, just a little filling to clean up. There is not enough throw in the original cam to extract long rifle shells if the shell does not fire. So keep this in mind. I can email you the spread sheet if you want. I may even have a picture of my set up if I can find it.
