It could be a root disease such as verticillium wilt but this usually outright kills a plant. Also, you don't mention if you fertilize the tree or not--possible toxicity from fertilizer applied too heavily? I lean more to a root disease since dropping leaves is an early symptom. It is frustrating to lose a tree which had been doing nicely for you. Wish I could be more helpful.
