I use the medium side also, but I just use a little more paint than I would normally use to make the dot. The paint spreads, thus creating a bigger dot. Also, if you use the v-rush pens (I love the blue one) you can use the little point on the top of the pen to make a big dot. If you use less paint, you can make a smaller dot. Hope this helps!
