Particularly with health problems, you want to be around family. You don't mention anyone. If you have family you're close to, then there's where you want to be.
I assume you've already checked out the health care system and have private health insurance that will cover you after you move to Ireland.
Why Mayo? I'm intrigued. But, again, health matters are going to be uppermost. That probably translates to urban/town/city. It possibly may need to be a substantial place with a major hospital.
Everyone in Ireland loves their own patch, so everyone's advice will be that their corner of the country is the best. Think support systems and go from there would be my advice.
