I wouldn't consider resuming a relationship with him until he undergoes therapy to determine why instead of speaking up about his unhappiness, he had an affair. This would not be a good basis for a marriage. He needs quite a bit more insight into understanding himself, so therapy for him should be a prereqresite for any further relationship with you.
