Depending on the publisher, an author hasn't too much influence on a book cover... A good friend of mine had a very, very intense 'fight' with her publisher about the cover - she won, but only because the publisher was so very eager to publish her book and my friend said, that she would withdraw her work if he doesn't agree. Of course, that's only one example - there are thousands and thousands others and one can't derive a rule out of this.
In Sarah Ferguson's case it could be the other way - she needs the royalty of the book sales, so I guess she accepts her publisher's wishes about the covers
