I don't think the problem is with Sarah, as she uses Sarah Ferguson. The majority sales of her books are in the United States, so her publishers are going for maximum publicity as the fascination with royalty and so add 'the Duchess of York'. Plain Sarah Ferguson doesn't have the same publicity value when looking at selling as many books as possible.
