Courtesy titles in the British system are strictly real titles. They're not called 'courtesy' because they're not created by any monarch ( as is the case in some other noble systems ), but because they're used by someone else than the real holder. If someone would be entitled to a courtesy title but doesn't have any at his disposal, they just have to make do without.
