According to Winick, who's veracity should be questioned at all times, Howard Porter hurt his hand and has to be replaced as artist on Trash of Shasham ... I mean, Trials of Shazam.
As for why the last parts of MSoE and Justice were late, blame DC and their distributors. I happen to know that Alex Ross had # 8 finished and that Jeff Smith had all four issues of MSoE done before the first issue of either came out.
