It sounds like the case split inside the chamber. Considering how the slide, barrel and frame go together, it's possible the guide rod spring broke behind the head, and the guide rod itself came off the barrel lever and is wedged in the breach. When the round was fired, the slide came back, the spring broke, and the guide rod didn't stay in place - when the slide came back to chambered position, the guide rod jammed in there. Take it to a gunsmith.
