I have been dealing with chronic foot licking, ear infections, and other skin issues with 2 rr's for the last 3 years. We tried different antihistimines, food trials and many other things. I have had them allergy tested and we tried simple ingredient kibbles to no avail. What does seem to help is a raw diet. If you have a holistic vet in your neck of the woods I would go see them, they do wonders. I would change his food right now to a protein and carb he hasn't encountered yet and try him for a few months to see if his condition improves. This is called a food trial. It doesn't help that much in my experience if he is also allergic to any environmental factors, which can be dust, cleaners, pollen, etc.
There are also several different types of antihistimes that work a little different than benedryl and the only one my guys seem to respond to is zyrtec. A vet told me that less than 20% of dogs will respond to any antihistimine.
Has he had any vaccinations the last few months?
