My last boy blew his ACL around the same age as Kota. Since he was active and healthy I decided to have the surgery done. It was not a pretty recovery - lots of pain for the first few days followed by a long structured rehab. The real bummer was that after months of by-the-book rehab, he then went out and blew the reconstructed ligament (long rabbit chase through the sierra snow...so at least he had a good time doing it!). The surgeon offered to redo the surgery for free, but I did not want to put him through that misery again so I passed on it. I just accepted the fact that we needed to cut back the activity level (no more backpacking or running, but plenty of swimming) and let him live out his days at a slower pace.
In short, the surgery is a major event for both you and the dog with no guarantee of success. I'd make the judgment based upon the dog's age, overall health and activity level.
