spitting up through nose

I have a four month old who has PRS. Approximately 10 minutes after I put him on his tummy he spits up and it comes out both his mouth and his nose. It looks like it hasn't even been digested and is straight out of the bottle. I make sure that tummy time is at least 45 min or more after he eats and that he has burped good. This is the only time that he spits up with it coming out of his nose. I am worried because he acts like he is choking when this happens. Is this normal? Can you tell me why it is happening? Do you have any suggestions to prevent it so he can still have tummy time?
