I was going to mention hypotonia before even reading through the entire post. I'm not sure the correlation between PRS and low tone, but it's a definite possibility here. When you pick him up, under his arms, can you feel his back and torso strength, or does he feel as if he's going to slip through your fingers?? It can't hurt to ask at his well check. Your ped will be able to tell and can direct you.
