I wonder if your seasonal planting/repotting routine is causing the slow growth.
I got a nice all-green 3-gallon Med Fan a couple years ago. It's been a vigorous grower for me (always potted). One time I potted it up to a larger container. It temporarily pouted and pulled spears. I was kind of surprised, but I knew that some palms do this. Once it regained its footing, it was off to the races again!
I may have had this palm for 3 years. It has easily doubled in size. Its trunk is about two-thirds the size of yours. But it must be far younger than 19 years old.
I have it in Miracle-Gro Palm & Citrus soil. I occasionally give it some lime & potash. Most Mediterranean plants respond well to those treatments.
