No, an official visit is one below in terms of precedence. No-one makes state visits to the GM of the Order of Malta, but neither do they make state visits to the Pope. All the ambassadors accredited to the Order of Malta are also accredited to the Pope (although not vice-versa) so it is customary when a head of State whose country is accredited to both visits the Pope on an official visit, to also call on the Grand Master. When the Pope travels he either comes in a purely pastoral role (when he went to Lourdes, for example), on official or a State Visit. Next September when he comes to the UK he will be the first Pope ever to stay at a British royal residence - but we do not yet know whether this will be treated as a state visit (Papal flags on the Mall, Carriage Procession, etc); I rather doubt it.
