Czar Ivan V, who was co-Czar with young Peter the Great, is an interesting historical figure. He was said to be physically feeble and mentally-retarded, yet he produced 5 daughter with his wife Praskovia (Saltykov) before he died, aged about 30. He never attempted to rule, and allowed Peter to fill the role of monarch. Peter was said to be fond of him.
I have no idea, though, who his doctor was.
